Extensive Cervicofacial and Retrosternal Emphysema Following Forceful Nose Blowing Associated with Non-Traumatic Maxillary Sinus Fracture: A Rare Case Report

This manuscript describes a rare case of extensive cervicofacial and retrosternal emphysema caused by forceful nose blowing in a patient without a history of facial trauma. Computed tomography revealed a displaced anterior wall fracture of the maxillary sinus with widespread air dissemination extending into the facial, cervical, and retrosternal spaces. The case emphasizes the importance of considering occult paranasal sinus fractures in patients presenting with acute facial swelling and subcutaneous crepitus after Valsalva-like maneuvers. Early recognition and CT imaging are essential for accurate diagnosis and prevention of potentially serious complications. Given the rarity of this presentation and its educational value for emergency physicians and otolaryngologists, we believe it will be of interest to the journal's readership.
